Question forms in the Past Simple

There are two main types of question sentences in English. The first is called yes/no question and the second is called information question. People sometimes call the information question w-h question, because they always start with w-h words like what, where, etc. People use information questions to ask about the information of a person or a thing. When people are not sure about something or somebody, they always use yes/no questions.
Yes/no questions begin with the verbs do, did, does, to be and the other helping verbs in English. When people ask the yes/no questions about the past, they begin their questions with did. To form the questions in the past people speak as bellow:
Did (helping verb) + you (subject) + go (Basic Verb form) + to the hospital this morning (…)?
See the situation below to understand how people ask and answer the yes/no questions.
Jack                : Did you know about John’s problem yesterday.
Janny             : No, I did not. What happened to him?
Jack                : He got a car accident.
Janny             : Oh! My god. I did not know about that. Did you go to see him in the hospital?
Jack                : Yes, I did. I went there yesterday.
